Purpose & Instructions

Use this Violet Sheet to prepare for appointments about breast or reproductive health concerns. The ROOT Framework helps you document symptoms, communicate clearly, outline your needs, and keep track of next steps—so you can get answers, not assumptions.

Clearly describe your symptoms and concerns:
“I’ve noticed a new breast lump and my periods have changed. I’m concerned about cancer risk.”
Use specific, fact-based language about your symptoms and family history.
List any previous delays or dismissals.
Ask clarifying questions:
